Sally Hare – Solution Focused Hypnotherapy Consent Form to Receive Solution Focused Hypnotherapy I (print name): Of (address): Telephone: Email: Hereby consent to receiving Solution Focused Hypnotherapy, under UK law and jurisdiction, from: Sally Hare DSFH HPD AfSFH MNCH (Reg) I understand that Hypnotherapy is not a replacement for medical treatment, psychological or psychiatric services or counselling. I also understand that Sally does not treat, prescribe or diagnose any condition whilst practicing as a Hypnotherapist. I understand that Hypnotherapy results vary on a case-by-case basis and that Sally does not guarantee results. I am aware that I am free to terminate any or all sessions at any time whilst observing cancellation policies in place and I have agreed to participate in each session to the best of my ability and of my own free will. I understand that absolute confidentiality regarding the content of my sessions will be kept by Sally Hare unless I declare that I or others are at risk. Appointments cancelled within 24 hours will be charged the half rate for that appointment, apart from Initial Consultations which will be charged the full rate. Sessions missed with less than six hours' notice, or no notice, will be charged at the full rate. All timely cancellations will not be charged. If you are late for a session I will still see you if possible, but will finish at the pre-allotted time so as not to inconvenience later clients. This may mean your session is shorter than usual but you will be charged at the full session rate. No refunds are given subsequent to the completion of hypnotherapy sessions. Online sessions Clients are reminded to safeguard their own privacy during online sessions. Please ensure you have an adequate internet connection for online therapy. While the occasional short lag or glitch is an occupational hazard, I have a fast ethernet broadband connection and can only give refunds for poor connection if the problem lies with my system.
